Due to our COVID Restrictions we are asking all potential patients who wish to register to first check that they reside within our Practice Boundary
If you live in our practice area and wish to register with the practice, please complete all the relevant forms and email us scanned copies. A list of forms can be found here; as well as details about adult /child registration and where online access to your record is explained.
If you want online access to your record then along with the completed form please provide a recognised form of photo ID (e.g. passport or driving licence) and proof of address.
Elements of your health record can be accessed by NHS Partners for research and planning purposes if you wish to 'opt out' then please notify NHS Digital
You will be offered a health check with the nurse or healthcare assistant. This allows us to ensure that we are fully aware of your present medical requirements and to offer advice regarding other services available at the practice.
Everyone is entitled to register with a GP; if you are homeless, an asylum seeker or in doubt please ask a member of staff who will guide you through the process.
If your details subsequently change during your time as a registered patient with the Practice, including: disability, Carer status, marital / civil partnership, trans gender status or any other information which would enhance the way we are able to provide support and healthcare to you then please notify a member of staff in person or write/email us.
